Call for Poster Proposals

Deadline: 5 p.m. PST on December 31, 2008

For the CAHQ 2009 Annual Spring Conference

March 30 & 31, 2009 at the Hilton Hotel, Costa Mesa, CA 

The CAHQ Education Committee invites you to submit poster proposals, highlighting quality improvement projects, to be considered for the 2009 Annual Spring Conference. All submissions undergo peer-review. Please carefully read the following instructions to ensure your proposal undergoes consideration. Please note all due dates. 

Each Poster Presentation Proposal must include:

  • Improvement Project Title
  • A Short Description -Write a brief one sentence description of the improvement project. It should clearly state what the team improved and the results achieved.
  • Organization Name & Address
  • A list of the Improvement Team Members, with Full Names, Degrees & Job Titles. (Spell out acronyms)
  • Aim of the project in 1 sentence.
  • A list of the measures tracked in the project
  • Description of the changes tested during this improvement project. List each change as a bulleted one-sentence statement that begins with an action-oriented verb in the past tense. Provide enough detail so users understand what the team did & what the specific change is.
  • A description of the actual Excel graphs & the data they plot. (You may include the actual graphs, if you desire, in the proposal. See information below about what is to be included in the graphs on your poster.)
  • A short (no more than 4 sentence) summary of the results.
  • A short bulleted list of lessons learned
    • Each should begin with an action-oriented verb
    • State the lesson and why it was important and what the team might have done differently
  • One Key Team Contact Name with title, organization name, address, phone, & email

Upon Acceptance of the Proposal the following items are also required:

  • Disclosure Statement (Vested Interest) to be submitted prior to February 28, 2009.

Submission of a poster proposal implies a commitment to register for and attend the annual conference, to ensure timely set-up and removal of the poster presentation at the annual meeting, and to be available during poster presentation times to discuss your poster with interested attendees. The key contact person will be notified via email of proposal status by February 1, 2009. Please make sure your email address is correct.

The poster presenter(s) should register and pay for the conference prior to February 28, 2009

Guidelines for Poster Presentations

  1. If possible, use a poster measuring 36” high, 48” wide (if using a tri-fold board, the folds are approximately 12”X24”X12”. Also be sure you have a way to attach the poster to the bulletin board.)
  2. There will be a bulletin board available to attach your poster to. You will want to bring a sign with your presentation's title, presenters' names and institutions to place at the top of the bulletin board.
  3. There should be at least one Excel graph plotting the data of the results over time.
    • Graphs must have the following elements:
      • A title reflecting the data being measured and plotted on the run chart
      • An X-axis title corresponding to the units of what is being measured
      • An X-axis interval (or scale)
      • A Y-axis title showing the unit of time over which the data is being tracked and measured
      • A Y-axis interval (or scale)
      • A numeric goal line
      • A Sample size
      • A Change annotation with a number that corresponds to when they were tested or implemented. The Change number corresponds to a brief description of the Change below the graph.
      • A Legend if there is more than one series of data plotted on the graph.
  4. Bring an adequate number of business cards to give to participants who would like to contact you for copies or additional information when they return home.
  5. Any handouts and other information from your poster presentation will be published in the conference handbook. Please prepare the information so conference participants have the same information as presented in your poster, plus any additional information/explanations you want to provide. This information must be received by February 28th, 2009, to be included in the conference materials.
  6. The poster can be most effective for your presentation and most valuable for the conference participants if:
    • All lettering on the display board is legible and large enough to be read from 5-10 feet away.
    • The poster clearly and concisely states the theme of your presentation, such as a statement of your topic or problem, major findings, conclusions or strategies to be discussed, or any other pertinent information. The sequence of information on the poster should be logical and clear in order for a participant who just walks by and reads your poster to have a clear idea of your presentation and the major facets of it.
    • Use lists, phrases, bullets, charts, drawings, or photographs on your poster instead of full text blocks. This is will be more appealing to the eye for participants and full-text material can be included in the printed conference materials.
    • Use no more than 2 or 3 colors on your poster for eye appeal.
  7. Reminder: equipment using electrical support or other audio-visual equipment MAY NOT BE USED in a poster session.
  8. You must bring any supplies you may need (tape, felt-tipped markers, tacks, a way to attach your poster to the bulletin boards, etc.) as we will be unable to provide supplies to the presenters during the conference.
